Exercise 2.4 Diabetes affects a growing number of Americans. An APRN working in a local hospital is part of a collaborative of community agencies strategically addressing diabetes from a community perspective.
Diabetes is a worldwide concern, with patients being diagnosed at alarming rates. Lifestyle choices and socioeconomic status are significant contributors to diabetes. Some social determinants of health the community should be aware of concerning the risk of diabetes are income, housing, education, occupation, access to healthy food, and physical activity. Several studies have been explored and describe race, ethnicity, poverty, food environment, and business culture to be factors where there is an increased population with diabetes (Myers et al., 2016). Overall, the common denominator in social determinants is education. The lack of resources and ability to understand the disease process is the common social determinant that APRNs must alleviate to help care for these patients.
There are many resources the APRN could use to identify different outcomes related to diabetes. The internet has many helpful resources, but the most beneficial resource to identify disease-associated programs is Healthy People 2020. The CDC website also had excellent resources for identifying programs and education services for diabetes. Some of the ones I have found are the National Diabetes Education Program (NIDDK/ NIH; DDT/CDC), National Diabetes Information Clearinghouse (NDIC), American Diabetes Association (ADA) Education Recognition Program, and Association of Diabetes Care and Education Specialists (ADCES).
Diabetes is considered a preventable disease risk factor of obesity, causing treatment costs in the United States of three hundred twenty-seven billion dollars in 2017 (Curley Cupp, 2020). There are many health concerns and long-term effects of diabetes that should be of interest to the community. Diabetes can damage blood vessels, causing heart and brain complications and damage the kidneys, eyes, nerves, and peripheral limbs (Amutha & Mohan, 2016). Wounds may have delayed healing, causing potential issues with amputations of limbs. The increase in health complications causes concern for the community because of an increase in sedentary lifestyles caused by complications of diabetes. Increasing the awareness of potential risks of diabetes can help many people turn the disease around and make better decisions for the future.
The AHRQ’s Healthcare Quality and Disparities Report Data Query have a significant amount of information at the state and national level. Reviewing information available about diabetes, information regarding treatment related to diabetes care and regulation is available on the state and national levels. Hospital admission information regarding long-term and short-term complications on the national level is available. Hospital admission information is available on the national level regarding uncontrolled diabetes without complications. This information is a great way to access trends and determine prevalence in a specific area versus the national level. This can help the APRN assess what areas of concern need to be addressed in the community where the APRN is working.
